For XTherma, XERF, and the whole wave of RF lifting devices, the answer is firmly the second — and that's not a criticism, it's the key to using them well. They all rest on a decades-old mechanism: deliver heat deep into the skin, and the controlled thermal stress makes it build new collagen and elastin, which tightens and lifts. What separates the devices is how evenly and comfortably they deliver that heat.
